Official medical examination - appointment as a civil servant
Prospective civil servants must submit a medical certificate from a public health officer confirming their medical suitability for appointment as a probationary or permanent civil servant.
Description
As part of the recruitment process for civil service posts, applicants are required by their recruiting authorities to submit a medical certificate from a public health officer confirming their fitness for appointment to a probationary and permanent civil service post.
For applicants for a career at the 2nd or 3rd qualification level (QE) at the Bavarian State Tax Office, a separate procedure will apply from 7 September 2026. Please refer to Section 2 for further details.
1. Procedure involving an examination at the public health centre (for applicants to all authorities except the Bavarian State Tax Office)
- To apply for a medical assessment by a public health officer, please use the ‘Book an appointment’ button at the top of this page.
- Please refrain from making telephone enquiries regarding your booked appointments. If you encounter any problems with the booking, please send us a message to the email address provided in the registration form.
2. Procedure for applicants to the Bavarian State Tax Office
- The assessment is based on a self-disclosure form containing all relevant details from your medical history.
- Where possible, the assessment is carried out on the basis of the documentation provided, i.e. without an examination at the public health centre.
- Please use the following upload form for your application .
- The doctors at the Health Department will assess the self-disclosure form you have uploaded and the medical reports you have submitted.
- You will be informed in around three weeks as to whether
- the assessment will be drawn up on the basis of the self-report form or whether
- further medical documents need to be submitted, or
- an examination at the Public Health Department is required.
- If an examination is required, an appointment will be arranged for a medical examination by a public health officer.
Important note
- From 7 September 2026, a separate application process will apply to candidates applying for a career at the 2nd or 3rd qualification level at the Bavarian State Tax Office,

Required documents
To register for the medical examination by the public health officer: (does not apply to applicants to the State Tax Office; see below for these)
- Examination referral from the prospective employer (with a declaration of cost coverage, where applicable)
- Completed questionnaire
- Completed and signed assessment form
- Important: documents and medical reports from treating doctors, therapists and hospitals
Applicants to the Bavarian State Tax Office:
- Medical examination referral from the future employer, with a declaration of cost coverage
- Copy of identity card (front and back)
- Completed and signed self-disclosure form
- Important: Documents and medical reports from treating doctors, therapists and hospital reports
- Signed declaration of consent for the forwarding of the medical officer’s report to the State Tax Office
- Signed consent form in accordance with the General Data Protection Regulation (GDPR)
For the examination:
- Official photo ID (passport, identity card or driving licence)
- Original documents and medical reports from treating doctors and therapists, as well as hospital reports
Questions & Answers
In principle, the same strict confidentiality regulations apply to medical officers as to doctors providing treatment. The medical reports prepared by the department are limited to a summarised assessment of the examination with close reference to the questions posed in the order. The clients therefore only receive the examination results that they need for their personnel decisions. Everything that goes beyond this (e.g. diagnosis, own and external findings) remains in our records and is subject to medical confidentiality and data protection regulations.
