The Community Health Nurse promotes health in communities, advises on prevention, coordinates care, visits clients at home and strengthens self-management.

Course description

Our Community Health Nurse is available to you as a personal contact and offers support in the following areas:

- Assessment of health status and care needs and joint planning of suitable support measures

- resources

- Support services

- Coordination of home healthcare and care network

- Carrying out preventive and check-up examinations (e.g. blood pressure)

- Early detection of physical and mental complaints

- Prevention and health promotion

- Counselling, training and guidance (dealing with complaints such as pain, fall prevention, nutrition, diabetes, aids, care measures)

- Support and guidance for chronic illnesses and the transition from hospital back to everyday life

- Open nursing consultation hours: In the nursing consultation hour, the CHN offers you the opportunity to clarify your concerns and questions in a personal consultation without an appointment. Waiting times may occur here.

- Home visit: If you have difficulty leaving the house or need a home visit, the CHN will be happy to come to your home. This gives us the opportunity to discuss the next steps in your care and which aspects are particularly important to you.

Prerequisites

The programme is open to all people with support needs in Hasenbergl.

In the pilot phase, the focus is on

- Older people who live at home

- People with care and support needs

- Relatives who provide care and support

- People who have difficulties accessing the healthcare system

Treating clinics, general practitioners, outpatient care services, local authorities and other service providers are also welcome to contact our care specialists with care concerns.
